By clicking the Submit button on the
bottom of this page, I agree to the following terms:
AGREEMENT: I
hereby give my permission for the aforementioned player to participate
during the 2008 season in the designated SYA Baseball league.
I agree to abide by the rules of the league. I understand that I assume
all risk of damage or injury that may occur while my child participates in
league activities.
RELEASE: In
consideration of the Association permitting my child to play in the
league, and for other good and valuable consideration, the receipt of
which is hereby acknowledged, I, for myself, my child, and any other
parent, and guardian or any officers, directors, coaches, managers and
agents shall not be liable for any damages resulting from or arising out
of my child’s participation in Association activities and waive, release,
and discharge each of the foregoing from all claims, demands, damages,
rights of action or causes of action resulting from or arising out of my
child’s participation in Association activities. I have received
authority from any other parent, guardian or representative of my child to
sign this agreement and release on their behalf. |
